Instructions for the request of EU - Zootechnical Certificates for Embryos

1. Complete the required EU - Zootechnical Certificate - Submission Form
2. Complete the fillable PDF or attach a copy of the completed IETS D Form included with the embryo shipment.
3. Email everything to the appropriate breed association:
   1. Completed submission form
   2. Completed PDF or attach a copy of the completed IETS D Form
